Apply Birth Certificate for Newborn (Within 60 Days)
If you have just had a baby in Malaysia, you can register and get a birth certificate for your baby at any branch of National Registration Department (JPN) Office throughout Malaysia. There is no fee to be paid at JPN if you do this within 60 days of the baby being born. Documents required are:-
- Confirmation of birth form from the hospital (original);
- Prenatal card /maternity examination book (original and copy)
- Identity cards/ passports of the parents/ guardian
- Marriage or divorce certificate (originals and copies)
A MyKid is obtained for a Malaysian child immediately after birth. This card serves as a self-identification document for children under 12 years of age, acting as a supporting document for the birth certificate. It’s essentially a “MyKad” without the photograph and fingerprint. The MyKid is replaced with a MyKad when the child turns 12.
What if….your child birth certificate is lost, damaged or destroyed ?
The mother/father, owner of the birth certificate or an authorised representative (next of kin/attorney) may apply to obtain the extract of birth certificate by being personally present at the counter of the nearest NRD office and bringing along the original identity card or identity document and filling in Form JPN.LM12. Cash payment of RM5.00 is charged for every extract of a birth certificate.

Apply MyKad For Children Aged 12 (Within 30 Days)
Regulation 3 of the National Registration Regulations 1990 (Amendment 2007) states that a Malaysian child who has reached the age of 12 must register for the identity card. The applicant (child) must submit the application within 30 days of attaining the age of 12. Otherwise, a processing fee will be charged. The applicant and the parent/guardian are required to go to any NRD counter with the necessary original documents.
- Birth Certificate of the Child (Original)
- Identity card of Parent/ Guardian (Original)
- If the address differs from the parent/guardian address, it is required to provide the electricity bill/ water bill / sewerage bill / utility bill (Original)
How long does it take to process the MyKad application?
Applicants who choose to collect their MyKad at the Putrajaya NRD Headquarters and the Selected Branches with distributed printing machines may do so within 33 minutes. Those who choose to collect it at any NRD branch offices in Peninsular Malaysia will be able to do so within 5 working days. Any NRD branch offices in Sabah, Sarawak or Labuan will have the card ready for collection within 7 working days. This charter on issue of the MyKad applies only to applications by Malaysian citizens and applications that have no complications.

Replace MyKad for Citizens Aged 18 (Before 25 years old)
Regulation 18 of the National Registration Regulations 1990 (Amendment 2007) states that any person who has attained the age of 18 and is a citizen must have a replacement identity card made no later than the age of 25. Payment is RM10. Required documents are:-
- MyKad (Original)
- For the verification of address, it is required to provide the electricity bill/ water bill / sewerage bill / utility bill (Original)
